Apply less than 30 days before your extension expires. The Immigration Bureau does not handle ypour case earlier than that.

So if you visit them 3 weeks before it expires, you will have plenty of time if some documents are missing, nedd completion etc.

Just to make double sure - you are on a one year extension of a visa right?  You are not talking about a multi entry type visa valid for one year?  In the case of the latter you would not need a new visa until your last 90 day stay is up (about 3 months after the visa expired if you planned your trips well).
