My wife fainted after waking up

A few mornings ago my wife fainted. She woke up and walked to the bathroom feeling ill. Vomited and fainted. 

 

The night prior she had hives and I gave her an antihsytime called Hydroxyzine. She normally takes altarax and other brands but they didnt have this at the pharmacy and gave us H instead. I think its what made her sick and subsequently had an alergic reaction. 

 

The hospital run some tests. The following anomalies came back:

- Hematocrit HCT  36.9 Low

- Mean cell volume  77.7 low

- Mean cell hemoglobin 26.9 low

 

RBC morphology  'Abnormal RBC morphology seen. see comments below'.  

electrolytes

- chloride 111 high

- TCO2 17.3 Low

 

liver

alkaline phosphatase  21 low

We are going to see a cardio dr tomorrow in bkk. Evidently her blood pressure dropped and she passed out and it was very scary to see this. I had to throw water in her face to wake her up. 

 

The dr said next time we need to elevate her feet higher than her head to get oxygen to the brain

 

Do you think she should be seeing any other type of specialist? Should she be having a brain MRI?

 

Recently we had to get a visa from my mother country and they needed a medical done/ chest xray. It highlighted an enlarged right lung. 

 

Any opinions are appreciated

Hydroxyzine and Atarax are the same so this is unlikely to be the cause.

 

She is anemic but not severely enough to make her faint. However the cause of the anemia needs to be ascertained.

 

I don't think you have the chest xray results correct, can you post the actual report. Unlikely to  have enlargement of right lung only.

 

Nothing you said suggests need for a brain MRI.

 

Cardiac workup makes sense and I would suggest to also see a hematologist for anemia work up.

 

How old is she?

Take her to the local government hospital.

They won't spend a fortune of your money doing pointless tests.

Jumping up from lying prone is likely to make anyone faint, heartbeat lags blood pressure, perfectly normal.

How old is she? 

 

You're correct. She is anemic as a further blood test at the bkk hospital confirmed. He prescribed iron tablets. The iron blood tests just came back today via email and the resutls are:

- TIBC 227 Low (range 228 - 428)

- serum iron 71  (33-193)

- UIBC 156 (135 to 392)

 

She has the genes for thalsamia as confirmed by blood test yrs ago. 

 

We want to find out what caused her to faint and nearly die in the bathroom that morning. The dr said she should do a 'tilt table test' at a cardio dr and an echo cardiagram test

 

I forgot to report above about her electrolytes blood tests were also low. She was clearly dehydrated on that night when sleeping. 

 

Will see what the cardio dr says
