the answer to the second question is yes. Songkran ruined my passport. In the US it is $100 and I'm sure it's the same at any embassy.

At last check you could have as many sets of extra pages as you wanted as long as there is space to fit them in.

Rules changed a bit recently ... but I have over 4 sets in the passport I am trading in on Thus .... will ask them to add 2 spare sets then :o
