There's a seperate declaration form on the website.

I'm a little unsure whether my GF would need to sign it or not? The VAF1 form already has a declaration in it.

Sorry... I should have said. Its the UK VAC site.

A the bottom of the page it says

"Please note that effective 13th November 2006 along with each complete visa application form (except application form Oct 2006) a declaration (copy enclosed) also needs to be signed by the applicant and in case of a minor or a person under the age of 18 the same may be completed by the parent or legal guardian"

And then theres a .pdf declaration form that can be downloaded.

I'm pretty sure the "application form Oct 2006" is referring to the VAF1 but I'm not 100% sure so I thought I should ask one of you guys just in case.
