Tghe problem is that the more invasive tests/catheterizations they do, the more irritated it gets

I had a very irritated urethra in October after a cystoscopy at Bangkok Hospital in Chiang Mai, flexible cystoscope. Lasted for three or four days, burning sensation. In contrast, the stinging I get in Australia from a rigid cystoscope under sedation usually lasted about 8 hours. I'm just wondering if the lubricant or local anesthetic the doctors use here is responsible, and with multiple insertions the OP is in his situation because the irritation is profound.

Unfortunately they cannot help me at this hospital, I have just been staying here to get pain relief until the urologist I have spoken to in Bangkok is available, and he is tomorrow so somehow I have to fight through the pain from 6pm tonight when I will get my last injection of painkillers and the I'm on my own until I get to the emergency room of Bumrungrad Hospital.

At a guess he thinks it may be chronic prostitis which if it is they can treat, if not then I don't know?☹️

From what you have said in the past Pauly, I would think that very unlikely.

 

I wonder if a CT scan of the area and "old fella" would show something??

Just seen the urologist in Bumrungrad now and he did a rectal examination and he has found some complications through that exam.

He wants to do another cystascope on Thursday morning so I will know more after that.

What really bothers me is that the urologist in CM never did a rectal exam after I had complained of such pain?☹️

Are you saying that you have never had a digital rectal exam (for want of better terminology, a finger up the bum) whilst you have been undergoing these problems?

 

I ask this because it is a key element in diagnosing prostate problems, and indeed it is recommended that after the age of about 50, men have this exploratory procedure done on an annual basis, or thereabouts, in conjunction with a PSA test....a digital rectal exam (DRE) is a test that examines a person's lower rectum, pelvis, and lower belly, so can uncover any other problems which may be in the area.

 

I will be interested to see what the outcome is for you Pauly.

Yes I had the rectum exam before buddy, but not during my time with my current/previous urologist.

He just kept reiterating that he didn't know what was wrong with me.

Ask him to prescribe some Phenazopyridine to take after the procedure.

 

 As it will otherwise temporarily worsen the urethral pain

What are the implications of a painful rectal examination?

 

How serious is it.

What could I have wrong with me?

I had an MRI 2 months ago and it was clear and I was suffering a similar pain then.

I will ask the doctor when he comes but when he examined me I was in too much pain to ask the right questions,as can happen

Since you are having pain to begin with it is not surprising that the exam was painful, it further aggravated whatever it is that is causing your pain to begin with.

 

Not possible based on this to make any guesses as to what the problem is.

I had the operation 24 hours ago and the doctor said that he found a lot of debris as well as scaring in the uretha and has now inserted a catheter which will give the stricture time to heal.

I will have this for an indefinite period but he had said that as soon as I can survive on home medication I can leave.

The issue right now is getting off the Pethadine which I have been taking continuosly for a week 

I am having the last of it now, after that I can probably expect withdrawal symptoms unfortunately.

But he has said there is no reason why I should not fully recover although the pain I have at present tells me otherwise l☹️

Good news on one front Pauly, inasmuch as the cause of your pain appears to have been found.

 

I walked around/lived with a catheter and attached bag for well over a week and it really isn't a big deal (I did wear long trousers though!) so good luck with the outcome.

NOT opiates.

Luckily I didn't have any significant discomfort from the catheter, just a little uncomfortable at times, but there again I was very careful and became used to it.

 

It had to stay in place because there was no other way I could empty my bladder, and it did seem to work. However a short time afterwards I had to self catheter three times a day to get the bladder "working again" and used to being emptied (retrain it as the specialist said), and that worked, and even though it sounds awful to some people, it was a case of having to do it and I got used to it.

 

Having said that, bladder emptying doesn't appear to be a problem with Pauly, so once the urethral tissue has healed/is healing, then the drugs can do their work as you say.

 

I wonder why there was "debris" in the urethra, when his other specialist had given him a cystoscopy twice I believe, so surely that would have been noticed?

Not only debris X, also a lot of scaring.

The doctor this morning did mention that I may have to self catheterise, I had great flow after the second cystocopy, but that stopped and at the death I was once again squeezing it out. The doctor said that once again he had widened the uretha once more so we will have to see what the finished article comes out like.

I have had troubles with the medications, the pain doctor took me off Pethadine ( right decision I can't go home with it) put me on methadone but I immediately had a bad reaction to it, an opioid receptory depression episode.

It wasn't serous and I am fine now but will just have to get by on Arcoxia. 

Tonight should be fun☹️

Good luck Pauly and if you do have to self catheterise, remember to use plenty of Xylocaine gel, and give it a few minutes to do its job, before inserting the catheter.

May I be so bold as to ask if you are having a TURP??

 

I had one about 10 years ago and had a few hiccups along the way shortly afterwards, but the whole thing isn't painful and is nothing to be feared, so good luck if you are having one.

Moving in that direction.....high PSA...looking at PSA velocity.....3rd Jan is the Big Crunch.

 

If it is down just monitor treat/with drugs....same or up it will be biopsy and......????........in the lap of the gods.

 

Was offered a TURP three/four years ago, then doctors said leave it a while or you might need to have another before you die!!

 

Not a happy camper at the moment.

Let's hope it's down and they can treat it with drugs.

 

A friend of mine here has had his PSA up to 11, and his seems to swing about quite a bit, but now it is down to 5 his specialist has said they will keep a "waiting watch" on it, and he's quite happy about that.

 

Yes it's quite possible to have a couple of Turps in one's lifetime, as we get older, and I think I'm heading that way myself after mine of 10 years ago, however I'm trying to stave it off with various remedies and they seem to work, but with varying degrees.

 

By the sounds of it you may well be having a prostate removal, and if you are, then make sure it is done with the da Vinci machine and by an experienced operator.

 

Hopefully it won't come to that, and may I wish you all the very best of luck.

Thanks for that...much appreciated...sat yesterday transfixed watching their video on the da Vinci set up......hope it won't come to that.

 

I'm at 7 at the moment and hoping recent 'events' (pain/poor urine flow) are related to a transient infection, frequent sex and excessive exercise.

 

On various medications now to shrink the prostate and battle the pain (discomfort really).....and some frightening antibiotic....(hallucinations, paranoia, sever mood swings...so far nothing has hit me).

 

Really could do without this!!!

As Xylaphone says it's important if you do have to do it get a good surgeon.

I consulted Charempong at Bumrungrad to get my first diagnosis, but when came the time to have the TURP operation I went to a hospital in Chiang Mai where I live because it was half the price.

Biggest mistake of my life was not sticking with Bumrungrad, the saying "you get what you pay for" could not be more true, I have had 3 poor procedures up here and now Charempong is trying to sort the mess out, and of course it's probably cost me twice as much as it would have if I had just had everything done by Charempong in the first place☹️

OMG.....horror story......I will keep his name in mind. 

 

Hope he gets you back up and running on all 4 cylinders....asap.

 

I have this notion that if it's bad ......do nothing and burn the candle from both ends for whatever time is left.
