Bang Lamung Vaccine Registration with Passport Number - Online via google docs

The facebook page linked below has the info in the attached screenshot, as well as a QR code to scan which will bring you to a google doc form to fill out.  I was able to submit the form using just my passport number.  Supposedly, someone will call you back to confirm an appointment.  If that happens i'll provide an update.
