Since the requirements for ThailandPass test & go for Phuket differs from Bangkok and I couldn't find any useful information about entry to Phuket I would like to share my successful application for entry to Phuket.

- copy of main page of passport

- certificate of vaccination (1. and 2. shot if applies), to accelerate approval process, crop the QR code on the certificate and submit 

- proof of SHA + hotel booking, booking reference from Agoda is good enough

- proof of required insurance 

 

And that's it, no need of proof of pre booked PRC test, transfer to hotel, certificate from hotel or merger of certain certificates. 

Note: all documents must be in jpg format, while you follow the application it will ask you for more details of your passport, arrival date and flight number. If your certificate of vaccination includes a QR Code, scan and crop it to accelerate the approval process. 

I was approved and received the QR code for ThailandPass within seconds 

Same for me, Applied now 2 times for had the approval QR code in seconds.

Works great.

I only used a gmail email account instead of my own email server as I heard that this could cause problems sometimes.
