I've found forms which say Vientiane and Savannakhet.

 

Is it important to use a specific form? I am planning to cross and the Thai-Lao Friendship Bridge #3 into Thakhek but haven't seen a printable form online for this particular crossing. I'd like to have it done ahead of time.
