My plan is to apply the for the multi-entry and stay about 6 months. Two months stay One month extension Trip to Malaysia Two months stay One month extension Question is about the "Date of Departure from Thailand". Would that be the date we go to Malaysia or the final date we leave Thailand after about 6 months? Also, I've put "Duration of Proposed Stay" as 170 days as the total number of days excluding the Malaysian trip. Would that be ok? Thanks.

Don't really understand your question as the form asks for details of first entry/departure and second entry/departure so you have to state both departure dates. Note that I just made up my flight details on last two SETV from Sydney and just put 60 days stay both times even though 30 day extension was always planned (no proof of flights is required so it doesn’t really matter). You could put your actual planned dates for the two entries with 170 days stay or you could put two entries at 60 days each for 120 days stay. Neither will be a problem and you are not held to any dates once visa is issued (I chose to not include my extension as seemed less complicated) 1

I was ignoring the second entry for simplicity as it says, "For double - entries visas only".
