Extensions from a non O-A based on marriage do not require insurance. It's based on his ORIGINAL visa before annual extensions. It will be either an O or an O-A visa. If O, no insurance needed. If O-A, insurance needed. O is definitely better to the point where many people with O-As have started over with an O. 1

Surprised you have not exited (border bounce) without reentry permit to kill off the non O-A Enter visa exempt to "start over" No insurance required. As @Jingthing suggested this. But would you not first have to have done 1 extension based on the reason for issue of the visa, (over 50), before you could change to an extension based on marriage ?
