They are on here (Lamphun), if useful for Jomtien is anyone's guess.....

I did my retirement extension Tuesday, Apr 29th, and was handed the STM 2 Form to fill out when I was having my documents checked. Took about 1 minute to fill out while I stood off to the side.

 

I downloaded the form from the link posted by upnotover and converted it to PDF. I was going to attach it but it is not allowed to attach PDF files.

This comes up occasionally.

Don't understand why folks don't just fill em in on the spot at immigration.

At most offices they hand them to you. Basically it's a signature and fill in name. 

One minute done.

At CW the doc check chicks hand em to you straight away.

I sign the form while they check the rest of the docs.

The forms are also on hand at io desk.

STM2 form is the 'Acknowledgement of terms and conditions for permit of temporary stay in the Kingdom'.

STM9 form is the 'Acknowledgement of penalties for visa overstay'.

These are standard forms that I've been completing for years, nothing new, and can be downloaded from Immigration websites.

 

STM11 ?

 

The 'S' prefix denotes it's a form of 'statement', which you should read, acknowledge and sign.

because we know Jomtien could just use it as an excuse to send you to the back of the line, hence why most sensible people prepare all docs in advance

The acknowledgement forms have been required for some time

You can download and complete these forms prior to a visit to Immigration.
I've been completing these for a number of years, they are not new.

 

Perhaps it's the fact that you use an agent, you were previously unaware of these forms.

I went to renew my Non Imm O Retirement Extension - Jomtien yesterday.  It was my 20th and quickest ever!  I used the link to download the forms & filled them in at home.  I didn't do any of these - 

stm2/stm9/stm11.  I went at 1.45, queued for about 20 minutes to get my ticket.  Went to desk 8(?) & within 5 minutes my docs were being scrutinised.  The ticket guy had sorted my docs into a specific order but the scrutineer did a re-sort.  He rejected some papers but did not give them back but placed them on his printer for re-use!  He gave me a copy of something to sign (maybe one of the above - I didn't see the number) and I was home-free.  I left wondering why they can't stamp your extension there & then; why is it necessary to return at 2pm the next day?

It was an IOU for 20,000 BHT, but you failed to read it.

I did my renewal last week at Jomtien and they also asked for a copy of the landlords ID....first time ever in 12 years of renewals,when I mentioned that I had never needed this before the guy said...new boss here now he say you need to show.....just more complications that they come up with hoping we will go down the using agent route....they make nowt out of us 1900 baht chaps 👍

i did mine a few weeks ago and like you i didn't provide any additional forms, but at the end the IO handed me a couple of forms to sign, said nothing, no time to read. I reckon those forms were the ones mentioned in the op
