I think he's in been in the state of decline for years now, and at best he's semi senile, at worst he's quite insane. However his supporters seem to be in quite a state of denial. 

 

I refuse to put on winter tires because:

Compare Trump to Biden or the cackling idiot Harris and it's clear who the more coherent of the three are. When it came to Biden it was all "he has a speech impediment", when it came to the cackling incoherent non answers from Harris it was all "but Trumps a Nazi", and now with Trump, who has his own individual style and there's no doubt about that, it's Trump having "mental decline".
