NHSO Approves Hormone Services for Transgender Patients

In a landmark move, the National Health Security Office (NHSO) has announced the introduction of comprehensive hormone services for transgender individuals in Thailand, offering both pills and injections. This initiative stems from discussions between transgender activists and was endorsed by the previous prime minister, becoming a significant policy under the current government.

The hormone services package, greenlit by the NHSO board in July, comes as a collaborative effort involving transgender groups, medical professionals, and the NHSO. This initiative aims to provide tailored medical support to transgender individuals, with an extensive consultation process to ensure informed decision-making.

The hormone services package includes a variety of treatment options to cater to different needs and transitioning stages. For those transitioning from male to female, younger individuals might use hormone blockers, whereas oral medications may suffice for older individuals. The physical effects range from wider hips to softer voices and reduced facial hair. Conversely, for female-to-male transitions, injections are typically used, leading to changes such as a deeper voice, facial hair growth, and increased muscle mass.

Transgender individuals seeking these services must undergo thorough consultations with experts, including doctors, nurses, social workers, and psychologists. This process ensures that individuals have a clear understanding of the treatment's benefits and potential risks, which include allergies, cardiovascular concerns, and more. These consultations also aim to assess genuine gender dysphoria and identify other health or psychological considerations.

To support these services, a budget of 140 million baht has been earmarked, covering necessary medical assessments, such as blood tests and cholesterol checks, scheduled at regular intervals. If service demand surpasses expectations, an increase in the budget is planned for the following year. Presently, medication procurement is underway in Bangkok, with existing clinics continuing to offer their services. The Ministry of Public Health is devising models to expand these services across other provinces.

The NHSO is prioritising the rollout of these services in locations where they are already accessible, particularly in medical schools in major cities. Telemedicine will play a crucial role in consultations and ongoing monitoring, particularly for patients with stable hormone regimens. In a positive message, Nithiwat Saengruang, Assistant Secretary-General of the NHSO, highlighted that this initiative offers hope and improved healthcare access for transgender individuals throughout Thailand.

The NHSO plans to soon release a list of hospitals and clinics where these services will be available, furthering accessibility and awareness among the transgender community. This step forward underscores Thailand's commitment to inclusivity and comprehensive healthcare.
