Retirement visa renewal - additional form?

Hi....I got back from Jomtien this morning after taking my usual documents to renew my 1 year extension/retirement visa and the young lady at the information desk checked my documents which were all in order but then she filled out an additional 2 page document that i then signed on each page. I took this along with my other documents to counter number 8 as usual etc and the guy took them and also took the additional document the young lady had filled in. My quick questions to our members : is this a new document we have to fill in ourselves along with our normal documents for renewing the visa and if so is it available for download somewhere..........Many thanks in advance and hope everyone is enjoying their day :)

Forms for extensions based on retirement which you can download and complete beforehand are currently;

TM7 - Application for Extension of Temporary Stay. 4x6 photo attached.

STM2 - Acknowledgement of T&C for Permit of Temporary Stay

STM9 - Acknowledgement of Penalties for Visa Overstay

STM11 - Consent Form for Fact-Checking and Information Verification of Application for Visa or Change of Visa Type or Extension of Temporary Stay in the Kingdom

If you require a re-entry permit - TM8.

I always go 2 weeks before and ask them what I need. I am very lucky my Immigration office is small. They always give me 2 forms to fill out and sign for when I do my extension.I live up north.

Well now they'll give you 3 forms.
The new STM11 was only introduced earlier last year.

Hi Liquorice,

I asked for the visa extension (retirement) at Chaeng Wattana last week. I got the 4 forms you mentionned above.

The last few years I also got a ''Statement'' to fill in. Is this ''Statement'' replaced by the new STM11?

Also, do we still need to sign the form ''Criteria for consideration of granting ...''?

For retirement;
TM7 Application for extension of temporary stay in the Kingdom
STM2 Acknowledgement of terms and conditions for permit of temporary stay in the Kingdom.
STM9 Acknowledgement of penalties for visa overstay

Applying for extensions based on Thai family, previously you were handed a form titled 'Statement', which has now been replaced by form STM10. - Affidavit of Family relationship.
That should only be completed by a family member and is not applicable to extensions based on retirement.

For extension based on Thai family it's as above for retirement + form STM10.

Anal, so it missed the S,🤣 The other form attached together to the STM2, was the STM9 Penalties for a Visa Overstay.

The form ''Criteria for consideration of granting ...'' doesn't have a header or a number. It is a document that lists the conditions you have to fulfill in order to obtain the extention (age, financials,...). You only have to sign it.

The 'Statement' I mentionned above, indeed seems to be a document for extensions based on Thai family. I've been filling it in for many years now, even though I don't have a Thai family... It never made any sense to me. :) And immigration never told me I didn't need it. TIT.

So, for me, 'statement' OUT, STM11 in.
