It's a mental issue for sure, but it is motivated by his overwhelming desire to love a woman and be loved by a woman. Because this desire was frustrated in real life in the most traumatic way his "romantic libido" so to speak, not his real libido, but the romantic equivalent, wants to love even the most debased woman, the prostitute. For if he can love a prostitute, he can then love the woman who inflicted so much pain on him, and hold on to her, for she is better than a prostitute. But she was so awful that he has to love all women, even the most degenerate ones, Dworkin, prostitutes etc...in order to be able to do so.
