Just to comment that I have recently signed up with AXA International Healthcare - standard policy with cover for all areas except USA. They waive the first 2 months of premiums and then allow one to pay monthly via credit card.

For me (49 years old, reasonably healthy), the monthly premium is about 5,000 baht. So my first year of premiums = 50,000 baht (1,000 pounds), which seems quite reasonable, especially with the monthly payments.
