I have a friend who is here on a spouse visa, has she left her ILR application too late? the spouse Visa expires end June, the life in the UK test has been booked for end of May, if pass is it still poss to apply for ILR? With so short time being left on the spouse visa, if fail would she have to return to Thailand? I know they left it late to apply so there own fault really but just though i would ask what options they may have?

No not really if she fails KOL test she applys again for FLR in the UK.

As long as the application is submitted on or before the day her current visa expires she wont have a problem.

When any application to extend one's stay in the UK is recieved one's current leave to remain is automatically extended until the new application is processed and the decision made.
