You've got the information you need for Ubon already, but I thought I'd expand on this to save anyone else from doing what I did and going to Khon Kaen. The passport office there is still closed because of the effects of the fire in the provincial hall. Currently the passport offices in Isaan that are open are in Ubon, Udon and Khorat. You have to complete the paper work and then wait for the new passport to be posted to you by EMS - which takes about a week. I went to Udon, which despite another gutted provincial hall nearby is operating in an adjacent building.
