I am an American living here in Thailand (five years with my American/Thai wife (30+ years)). My wife’s niece had a child (a boy now 3 years old) and he has lived in our home since birth (supported by my wife’s house registration). My niece and her ex-husband have parted ways and neither of them wants to care for him. Both have agreed to sign over custody to us. After this is done, we plan to go to the Thai Courts to adopt him. Has anyone done this? Any suggestions or help would be very much appreciated.

As a foreign national, you will have to begin the adoption process in the US, much easier if your wife becomes the child's legal guardian, this only require's the consent of the child's biological parents.

The link from animatic will be the starting point.

it will be an international procedure, but since it is a family adoption should be a bit easier. I am not sure it needs to start in the US, but both US and Thai law must be followed for the adoption procedure to be recognized by both countries.

Adoption by Thai law only has legal effect in Thailand. An international adoption makes travel easier but is more expensive.
