A good friend of mine is considering relocating to Thailand permanently. He has had chronic Hepatitis C for 20 or more years now. Is this a reason that would prevent him from getting a long term Non-O visa?

Well I only know about retirement.

For the O-A visa from home countries, there is a required health form but I don't think Hepatitis is listed as something to declare.

In fact, for retirement Thailand allows those with HIV so I doubt hepatitis would be an issue.

But you don't even need an O-A to retire in Thailand.

Retirement extensions require no health form at all.

The Elite visa is an option for anything under the sun, but is is not a suitable recommendation for the OP, ie it is off topic in this thread.

The non-B visa, single-entry or multiple-entry, would not be suitable. This visa is for a business visit to Thailand on behalf of a company located outside Thailand, nor for relocation to Thailand.

The only thing I would add is that your friend would need to pay for private medical treatment and medicine if it was needed.

This could be expensive - however if any medicines were needed you might be able to get an idea of the local price if you ask in the health forum.
