I just did mine yesterday. Absolutely painless!

Assuming you live in Chiang Mai, the office is the large building diagonally opposite, across the intersection from the Holiday Inn. ( 18.767122, 99.005588 )

Drive in towards the rear and you'll see a lot of motorbikes parked. Park there. On the left side, facing the building, there is a glassed-in office with three people sitting at computers. Simply walk in (no waiting) and look confused... One of the three will hold their hand out. You fill it with your Green Book. That is the only document you will need (unless you have some special requirements that you haven't mentioned.) They will motion for you to wait outside. The wait will be 4-8 minutes. About. Then they will call you back in and tell you how much to pay. I paid 240 baht. (It may have been 340 baht... I really don't remember. Small money, regardless.)

They will give you your Green Book with a receipt in it, and a little envelop. Take these inside the main office, turn left to the information desk, and show the lady your Green Book. She will give you a que number. When your number flashes on the Big Board, go to the window indicated on the board and give the person there your Green Book with the receipt in it. She/he will do some computer magic, and 15 seconds later you will be asked for 100 baht and given your new non-stick sticker. I had to wait almost 10 minutes for my number to come up. Your mileage may vary.

As I said... painless.
