First Non 'O' from Penang + extension of stay

I will be going to Penang in August to apply for my first non "O" based on being over 50, but I have a couple of questions regarding this and the extension based on retirement application.

I have downloaded the Penang Consulate application form in advance, and got my list of requirements ready. (Thanks ubonjoe)

For "Purpose of visit" I have ticked "Other" and entered "Retirement", but an not sure what I put for "Proposed duration of stay". Is the single 'O' visa valid for one year, and I put one year? Or only 90 days as I must apply of an extension of stay?

Basically I am asking, do I put "ONE YEAR" or 90 DAYS?

It seems that the requirement for this are different for each area. Can anyone please provide the current requirements for Samui Immigration?

I have read several threads on this,and the most recent indicates a medical including X-rays, but these things change.

My money has been brought in over many years. Always paid left over travellers cheques into my Thai bank. Will I need to provide some sort of proof the funds came from overseas?

I still have receipts for Travellers Cheque Exchanges. Will these suffice?

Multiple reentry permit.

Is a multiple re-entry permit, THB3800 I think, valid for a full year or only three months?

Duration of stay put 90 days since that is what the visa allows.

Samui immigration does not follow the written rule and does now require a medical certificate issued by a hospital according to the same reports you may have read.

There is no requirement to show the funds came from abroad. But it would it would not hurt to have the receipts for the travelers check exchanges with you in case you are asked for it.

The re-entry permit will be valid for the length of the extension.
