TM 47 in Chiang Mai requirements?

Looked on the pinned subjects but not there, did a search and only got doing it on line, so apologies for such a basic question. It's ages since I did one, and my memory isn't the best.

Anyway, what I know I need is :

TM 47 filled in

Passport with departure card

proof of current address.

 

Anything else?

 

I gather I don't need a photo, or have to pay to do it.

Nothing to pay, and no photo for the TM 47 form. You possibly might need a photo if they decide to have you fill out the separate foreigner information form, the latest bit of bureaucratic insanity being perpetrated by some immigration offices.

Thanks for reminding me about the info form. I hadn't thought about that.

You didn't mention any additional requirements, so I hope I can assume there aren't any.
