Has anyone using Koh Samui immigration had experience with the need to show TM 30 documents? How do I get it?

 

Also in the latest list of documents required for extension of stay for retirement there is a new one which says

12. A FORM OF VISA EXTENSION CONDITION (Imm.2)

 

No idea what that is so any help appreciated.
