Can you please give some places to have an health  check up in Bangkok ( or Prachinburi ), with bill in English ( for my health insurance which pays it 100 % every five years )

with PSA rate 

I would prefer to avoid big hospitals like Bangkok Hospital, rather small but good clinic

Medical care is normally provided by doctors in hospitals in open out patient settings rather than clinics; and larger hospitals will have their own testing facilities for all common tests and available within an hour or so.  You have the insurance so best to use a good facility where you are likely to have readings reviews by a competent doctor.  Clinics tend to be for beauty and medical types often not more than pill pushers.

 

Any major hospital should be able to provide bill in English (ask for a detailed receipt in addition to the normal cash receipt - this is what insurance wants to see).
