Only your passport, in which you have presumably stapled the returned portion of the form from your previous 90 day report. Even that is probably not strictly necessary.  Everything is computerized now. They do the rest.

It all depends on where you apply.

Some require the tm 47 form and copies of a couple of passport pages.

If they require a form then you will find one at the office (TM47).

Easy to fill.

Bring passport and departure card (TM8) which you got when entering the country.

Seem to be your first report? Which office?

 

If you want to be fully prepared you can download and fill the TM47 at home (either handwritten or electronically).

In this case you should have the slip from your last report.

Likely stapled to your passport together with the departure card.

Nothing at all was needed. I was in and out in ten minutes!
