Medical cert for driving license.

Went to my local DLT to enquire about renewal of my 2 year to a 5 year, was told I need the following docs:

Usual copies of passport pages.

Residence certificate.

Medical certificate.

 

 

Is there an official form for this medical certificate to fill in ?, lady ( I use the term loosely) at DLT wasn’t hanging around long enough to exchange pleasantries [emoji51].

 

If so, can anyone drop me a copy ?

 

If not, I assume the standard checks at hospital or clinic will suffice ?

Any clinic or doctor, Bht 100. It is not needed for 5yr to 5yr, so you CAN drive with tubercolosis, syphillis and terminal piles.

Okay thanks, although with terminal piles I think I’ll give up driving and stay at home sitting in a bath full of ice !!

An update:

 

Went to a local clinic:

Filled in a form with my passport details, got asked my height and weight and had my blood pressure checked. 100 baht !

 

I now feel happier nudging the old jaloppy out into the soi !!
