i am in the process of re doing all the forms in acrobat, but this one i used 2 weeks ago at bkk immigration and is 100% acceptable:

using adobe acrobat change the fields to your information and print.

if you save it to your computer, then next time just change the date and print it again.

But you might want to change the name field example to be in BLOCK LETTERS as is requested in your update version.

Most people probably only have Reader so will have to retype all information each time. But it will still be a lot better than our handwriting.

then we just need to be able to email it in
