If libido was lacking before, there's no reason why an operation would improve it since libido is mostly psychological, right?
Maybe consider TRT (Testosterone Replacement Therapy) but you should get blood work done first (PSA, testosterone levels etc) 
There are several clinics in Bangkok and Pattaya that offer managed programs but they're not cheap.

 

 

Thanks BP. Libido is gradually improving. As far as erection is concerned, I will try out the little helper called "cialis".
I am aware that Cialis lowers blood pressure. I may want to reduce or discontinue my blood pressure medication during "Cialis-Phases". Monitoring my blood pressure carefully during those phases and just see how it goes.

I would also like to share my experience with Rezum precedure done at BNH hospital. I was in and out of the hospital within exactly 4 hours.

During the procedure (Cystoscopie plus Rezum) I was not awake and catheter was in place when wakening up. I almost immediately experienced

some discomfort because of the catheter whenever moving.

 

The good thing is really that the treatment is outpatient and I could return home and was able to walk short distances.  It is highly recommended to 

buy a pack of adult diapers and wear them because of blood not only in the urine bag but dripping out of the urethra as well. A slight burning pain is to be expected from time to time.

 

Starting on the third day i experienced quite some pain when sitting on chairs or hard surfaces longer than 20 minutes. Sitting on my sofa or armchair was 

however possible without issue. 

 

After 1 week I was given the option to remove the catheter. However, at that time I felt  some pain and decided to wait another week, even as mentioned the catheter was very inconvenient and sometimes painful. Today the catheter was removed (close to 2 weeks after Rezum), during removal i had a strong burning pain lasting 2 or 3 seconds. I was told to wait in the hospital until after 2 times of urination to see if there are any problems with that. Urination was ok, but not particularly strong with lots of blood coming out. Some hours later the bleeding seems to be less. Frequency and urgency of urination is annoying so far.

 

Next appointment will be in 2 weeks and I will keep you updated. All expenses were paid by my insurance passportcard (luckily  i have in and outpatient coverage which is very expensive but since some time already I am glad to have it because of several issues and the Rezum coverage was confirmed within less than 24 hours).

I have been in Adult Diapers for more than one year following my last urethral operation , my urethra was damaged while having a TURP operation then about 6 cystoscopy ops at various hospitals, but I'm convinced it was the initial operation that did the damage as I was in a lot of pain that started in the weeks that followed and had so many more cystoscopy  procedures afterwards.

I've been to Bangkok Hospital  spent 6 weeks in and out of Bumrungrad , but they could not get rid of my pain without high doses of morphine  followed by methadone to get off off that.

It goes through stages of being painful and pain free  but I manage to releive the pain sufficiently with Ultracet now.

Morphine is anawful drug to be addicted too.

Methadone is not much better  but I needed it to get off morphine  and it took time ,but worked.

My family were convinced I woukd have to go into a treatment centre   but I never needed to.

 

I get a lot of UTI's probably because of the diapers being wet during the night before I change them in the morning , nothing much I can do about that.

 

I have tried Kegal exercises, they only seem to make things worse.

 

I've tried a new product  they are similar to feminine pads but the only ones you can buy on Lazada are too thin so they will not last the day before you need to change.

Waste of time.

So it looks like diapers for life.

 

I wish that the procedure you had was offered to me 4 years ago, but it wasn't.

 

I hope your recovery goes well and that you enjoy a healthy sex life and be diaper and pain free forever now

I always suspected this to be the end result for many chancing a TURP in Thailand.

Good to hear from you TP, although not under these circumstances, and I was wondering what had happened to you, especially after we communicated quite often via PM some while ago.

 

It sounds like you went through a nightmare, and I remember you were getting more distressed as time went on, especially as there was seemingly nothing that the medical profession could do for you to ease your pain and discomfort – – now I know why.

 

Although a TURP is quoted as being the "gold standard" operation, obviously things can go wrong and in your situation, did, but according to all I have read, it is a rarity, although my TURP wasn't without its problems.

 

I wish you all the very best for the future and to keep searching for things and ways that may ease your pain and discomfort. All the best to you.

I would like to post my update regarding my experience with Rezum . It has been 1 months since the procedure and 2 weeks since removal of the catheter.

After removal of catheter urine flow was weak with burning pain and quite some bleeding for several days. I wore adult diapers during that time and the diapers had to be changed twice daily because of all that blood. During the first few days there was nagging urgency and frequency to urinate often.

 

Within 8 days bleeding had stopped and frequency/urgency was very manageable. Urine flow had improved considerably, already stronger than before Rezum. Today urine flow test was done and it was good, doctor Suntchai said it was like textbook and further improvement should gradually come within the next 2-3 months. Most of his patients come to complain during the first few weeks because after treatment the prostate is swollen and usually leads to

more severe symptoms than before. This was not the case for me. Overall I am pleased since the end of week 3.

 

Risk to keep in mind for a while are possible infections  because the prostate is still inflamed and it is easy for bacteria to enter the area. And in rare cases the inability to urinate, mostly caused  by the inflammation in combination with infection.

That's good to hear how about the. Other big question getting the leg over is that side of the equipment operating at the correct levels if you know what I mean.

Did you request Dr Suntchai or did he automatically take you as a patient?

Do you remember how many cc your prostate was?

Am I understanding the only pain you experience is the catheter while moving about and peeing during and after its removal?

 My dr up here only performs TURPS  , I asked him about the  Resume and my prostate size , he says its ok to go to see Dr Suntchai!

I made an appointment with Dr. Suntchai because at BNH he is the specialist for Rezum procedure and urological surgeries . I went there to get a second opinion about my case. My prostate had a size of around 50 gr and he considered me as a not difficult case. I mentioned previously that I also had intense pain while sitting on chairs or other hard surfaces for several days (beginning on day 3 after the procedure).

How big is too big a prostate and not suitable for the Rezum procedure? Just had a look at my last diagnosis a few weeks ago and my prostate was 68.6ml.

Generalk guideline is <80 so size wise you would be a fit.

Yes I did.
Thai insurers may not be able to delve into your US medical history but I could be wrong.

You should get a urologist to evaluate your suitability for the Rezum procedure.
If your prostate is too large, he might recommend something else.


Acquablation works on a similar principle except it doesn't utilise vapour; just water.

It does have the benefit of being suited to larger prostates and also extremely targeted so there's less chance of permanent side effects like ED or EjD.
Sadly, it wasn't available in Thailand last time I checked.

A Thai insurer would likely not accept you.  But a Thai issued policy is not recommended and not necessary in order to be covered in Thailand.

 

An international insurer (expat policy) might accept you but would definitely exclude all prostate related conditions.

As I think I mentioned on another post, if I had my time again, and judging by the mixed feedback from those who have had the Rezum treatment, I would certainly opt for Urolift, even if it meant travelling to another country to have it done.

 

IMO it would be worth investigating an overseas hospital which carries it out (Singapore, India etc) and determining the cost of everything all up. After all, you have the rest of your life to consider.

 

Good luck in whatever you decide to do.

I met with Dr Suntchai at the BNH hospital June 15.

My first  impression was that he was

very confident, his English was very good! 
I asked how many procedures he had done , he replied 50.

He asked about my condition and I told him I’ve been on meds Diox and Finn for 8 years and that my urine flow was slow and that I get up every 2 hrs to pee at night.He asked how big my Prostate was , I said my urologist up here in Sakon gauged it by 3 fingers.

He suggested we do a ultrasound to confirm the size , it was 83 !

He said my Prostate is large but it would be no problem for the Rezum procedure and suggested that the catheter be in for two weeks after the procedure, to lessen the chance of urinating problems that are associated with the Rezum effects.

I scheduled the procedure on August 8 ! 200k and a 14k blood and heart tests 2 days prior to TheRezum procedure.

That is only slightly over the maximum recommended size so should be okay. Normally, treatment is for between 30 and 80g. Hope it works out for you. I am still happy with the results. Was it as effective as I'd initially hoped? No, but still a good improvement. If I went back in time, would I still have the treatment? Yes, for sure and will do it again in a few years if necessary.

Hi again GarryP, long time no hear from and I hope you are well.

 

Just looked at your post and it coincided with a friend of mine who is in Canada at the moment having prostate problems, and he tells me that the iTind procedure/device is now carried out at the BNH hospital by that very same Dr you mentioned, and he knows this because he's been researching his options when he comes back to Thailand!

 

Looks like there are three options at the moment, aside from the TURP, with Rezum, Urolift and iTind looking to treat one's prostate. And there is more information about these on the net, although none of them will help me now because my bladder appears to have stopped working and the good doctor at Bumrungrad couldn't do anything for me, and had no idea why the bladder wasn't working any more – – so a nightly insertion of the catheter is needed in order to get some sleep. And believe it or not, I've gotten used to it now.

But how does one know which procedure is the most appropriate for oneself, if a hospital/urologist does'nt do all of these procedures then you will only be advised of procedures that they can do.

My Turp procedure that i had carried out in January this year is beginning to prove itself not to be very effective as i'm urinating more now than i was immediatley after the procedure. I also believe my bladder is not emptying fully as i'm only squirting urine not an even flow and i can see me needing more treatment in the not too distant future. At the mo i think i'm ok as i don't have any pain which would indicate an infection that would require immediate treatment.

Any helpful comments would be appreciated, thanks.

You make a very good point, and it's one that I have been pondering for decades, and without going off track too much, when I had bouts of prostatitis many years back, it would often result in an aching left testicle, so much so that it became unbearable, and my doctor in NZ referred me to "the best urologist in Auckland", so I thought I would be getting some expert advice. 

 

But to cut a long story short, he said he didn't know what was causing it however he could cut off my left testicle. I likened it to me inadvertently walking into a brain surgeons office and because brain surgery was the only thing he did, he would recommend removing part of my brain to cure the pain in the testicle!!! You see what I'm getting at.

 

Sorry to hear that your TURP isn't proving too successful for you, as was the case after my TURP (but a few years further down the track) and a urologist here said that I had scar tissue around the bladder neck which was hindering the urine flow, so after some consideration I decided to let him do a bladder neck incision, which he assured me would be straightforward and not cause any side effects. However it did and I was left with retrograde ejaculation, despite his promises at the beginning that this wouldn't be the case.

 

In short, I can't offer you anything helpful because all of my experiences on this and similar procedures have resulted in me not being able to pass water in any way, shape or form (actually I do manager a dribble every so often), having to use a catheter daily, and having retrograde ejaculation, so not sure that I can be of much help.

Having said that, there are several tablets that can possibly help, especially if it is still the prostate "playing up" so it may be worth trying one or two of those, so have you thought of discussing this with your urologist, because it might work.

 

And the more I read on this and other threads, and take into consideration my own experience, the more I remember the quote by a top urologist who likened this part of medicine as being like, "a medical wasteland" even though we seem to be moving along a little more these days, what with the new devices and procedures becoming available, although not that available in Thailand unfortunately.

 

On another note, and as regarding "an infection" one of the things which has helped me, I believe, is that I am taking a tablet recommended by a couple of professors I was in touch with, and this tablet is called "Hiprex" and is a "urinary antiseptic" for want of better terminology, and helps prevent any bacteria from taking hold in the bladder, and since I've been taking these, I haven't had a UTI, which unfortunately were quite frequent before I came across these.

 

You can find them online, but I haven't found them here in Thailand, however I do order them direct from a pharmacy in Australia.

And the more I read on this and other threads, and take into consideration my own experience, the more I remember the quote by a top urologist who likened this part of medicine as being like, "a medical wasteland" even though we seem to be moving along a little more these days, what with the new devices and procedures becoming available, although not that available in Thailand unfortunately.

 

On another note, and as regarding "an infection" one of the things which has helped me, I believe, is that I am taking a tablet recommended by a couple of professors I was in touch with, and this tablet is called "Hiprex" and is a "urinary antiseptic" for want of better terminology, and helps prevent any bacteria from taking hold in the bladder, and since I've been taking these, I haven't had a UTI, which unfortunately were quite frequent before I came across these.

 

You can find them online, but I haven't found them here in Thailand, however I do order them direct from a pharmacy in Australia.

I'm hoping and praying i don't need another procedure but i'm trying to put myself in a position that if i do i am going to undergo the most appropriate procedure for my problem. You mention BNH hospital, i will do some research regarding BNH and Dr Suntchai but do you know if they carry out all the procedures that are available ?

 

I have taken most of the tablets available for enlarged prostate/BPH when first diagnosed but found they either did'nt work or i suffered severe side effects so no more for me. Thanks for the info regarding ''Hiprex'' that sounds like something i should be taking given my situation.

 

I really sympathise with your catheter issues due to your bladder ceasing to work anymore and wish you all the very best in coping with it.

I'm sorry, I don't know about that, but they are advertising the iTind procedure at a one-off cost of 70,000 baht, that having a time limit on it I think

I've just phoned them and they confirm they do Turp, Rezum, Itind and PVP, apparently they don't do Urolift or they did'nt understand me.

So all in all along with Dr Suntchai its probaly my best option if i do need another procedure carried out.

That price for the Itind certainly looks good if that is the most appropriate for my problem.

Have you ever considered travelling to another country to get the Urolift procedure done? Esp as a couple of other posters have had goood experiences with treatment in India, and as I recal at a hospital in Kolkata.
