Recently diagnosed with LADA Diabetes and looking for the best Specialist to help me manage it

As the post says above, I’ve recently been diagnosed with diabetes and I’m looking for the best specialist/hospital combination available in Bangkok.

 

Here is a quick summary of what’s happened so far:

 

Sudden diabetic symptoms mid December. Peeing through night, and blurred vision.

 

Went in for blood work at Samitivej. Fasted blood sugars 286, a1c 9.6%.

 

Been on metformin 500 mg x2 daily since Dec 23.

 

Moderate low to low carb diet.

 

Healthy weight and extremely active lifestyle.

 

Blood sugars under control at the moment, in non diabetic range.

 

Tested positive for anti GAD so Doctor said it’s likely I have LADA Diabetes.

 

Blood tests taken at Samitivej:

 

LDLs 186. Healthy triglyceride and HDL levels. Healthy blood pressure.

 

MRI shows no fatty pancreas or liver.

 

Next scheduled test: C peptide.

 

There is nothing particularly wrong with the Doctor I am currently seeing but as I will need to manage this condition for the rest of my life I’d like to start a relationship with the best Specialist, ideally one that is up to speed on all the best practices in managing this specific version of diabetes.

High carbs and junk food carbs raising glucose levels are symptoms of diabetes; high fat is the cause of most cases of diabetes. High fat consumption clogs up the insulin receptors which causes insulin resistance and then things get worse over time...

I disagree, but I'm not going to play endless ping pong with nutrition debates. 

There's a lot of misinformation in the nutrition field.

Do you have a Blood testing machine ?  you need one you need to test every day...  286 is high do you know what the reading was this morning ? after taking 2 x 500 mg Metformin since December... R U going down ? going up?

you need to test mornings [fasting] + To start with  before eating + 2 hours after every meal, after a while you will know what is rising your blood sugar levels

 

Everyone is different + different food reacts differently in different people..  as for There's a lot of misinformation in the nutrition field - I agree 100%....  2 Thai friends [one is 64 the other early 40's] have Metformin + Insulin every day = nothing to do with being overweight as both very skinny thin

 

diagnosed with Diabetes 34 years ago, remember back then BS was over 500 Fasting + a1c 15%.  but think I had it for many years before then,  Red Cross Hospital BKK, diagnosed in 2015 have LADA Diabetes and Insulin 2 x day was added, last few years keep around 95 - 120 every morning [102 this morning]   103 with A1c 6.1  last week Hospital blood work.

 

For reasons never worked out, the few odd times will suddenly drop I get a cold sweat and shakes = test BS and sure my BS is down far to low 60 or under....   + also on the odd times my BP [Blood Pressure] will suddenly drop like to 50/30

That's VERY harsh. You do know that LADA Diabetes is *not* related to diet but is an autoimmune disease like diabetes type 1, right? LADA Diabetes is NOT the same as diabetes type 2 which is often due to lifestyle. LADA Diabetes relates to issues with the pancreas / pancreatic insufficiency.

Interesting, i never heard of LADA diabetes, the things I've read are about 1, 1.5 and 2.

 

Edit: just read up on it 1.5 is same as LADA

Kluaynamthai Hospital in Klong Toei has a floor for diabetes patients. Used it when I first was diagnosed with diabetes type 2. Now i just use a local hospital for follow up visits.

You say there is nothing wrong with your present doctor.  Then you should discuss your concerns and referrals with that doctor.  You mentioned Samitivej.  I’m sure they have specialists there.  I personally would use Bumrungrad or BNH.  
In choosing a specialist .  I would look at their profiles.  See what type of advanced training they might have. 
And your decision where and who you go to will be based on cost and insurance if any. 
I don’t understand if you are just seeking the name of a doctor.  Why you are posting your complete diabetes medical history.  You don’t want decisions on your condition based on laymen opinions or debates .  And has nothing to do with a recommendation for a doctor. 
The type of specialists that treat diabetes are:

A diabetes specialist is called an endocrinologist. Endocrinologists specialize in the glands of the endocrine (hormone) system. The pancreas is the gland involved in diabetes. The pancreas produces insulin, and problems with insulin are what managing your diabetes is about.

At the present time I’m seeing an Endocrinologist at Bumrungrad high cholesterol.  She is good.  Easy to talk to. And has had medical training in the states. She has gotten my cholesterol in the normal range.

Anyhoo, when I was in Canada I bought myself implant sugar monitor. You wear it for a month and can check your sugar levels all the time, but they do have a recommended times in the instruction manual to correctly formulate the graph at the end of the month. I see it being sold on Lazada.

I asked them a few years ago, the implant needed changing every 14 days, set up for a years supply was something like 24,000 baht....  + Dr Fee and training

 

I just use the normal Accu-Chek monitor, the strips can be bought in lots of places, Red Cross Hospital Pharmacy ground floor 100 strips = 1,200 baht, around the same opposite in the 3 Pharmacies on Rama 4 -  normally buy on-line around 800 baht for 100 strips,  been using a Accu-Chek monitor for about 25 years

 

With NO needles pricks at all per day would be an Insulin Pump and Glucose Sensor. all in one ? but injection site must be replaced every 3 days ?

If you go to a specialist, you'll be on expensive pills and whatnot for the rest of your life. You can treat yourself naturally with diet and foods, if you prefer. This has been demonstrated. Maybe start off with Dr. McGregor at NutritionFacts.org, if treating yourself appeals to you.

I have no knowledge or experience with LADA.

I am type 2 diabetic. Metformin is the medication

of choice which can be supplemented with

additional medication, like Gliclazide, to 

improve the effectiveness. Maximum daily

dose of Metformin (slow-release) for Type

2's is 1 000mg twice per day. 

 

There is a lot of nutritional mis-information

out there. Fats are not the demons they were

once purported to be. Sugars most definitely

are to be consumed judiciously.

That is what I have due to a surgical procedure in 2017 where the overload of steroids put my pancreas into a deep slumper and shut my thyroid down. Surgery was a triple neck fusion which lasted 14 hours.  Now my Thyroid is back up and running with no meds but my pancreas is only at half of what's needed.

 

I take Jardiance Duo which is Metformin 1000mg with 12.5 mg of Jardiance, taken 2x per day, 1 morning 1 night. Lantus solo star injectable at night 25 units and now Victoza 1.8 injected in the morning. A1C of 5.6, morning BS 90 to 105 (Fluctuates) and after meal BS of 125 to 130 after 2.5 hours. 

 

Sucks but it is what it is and with it came plaque psoriasis from the Auto immune issue the surgery caused.

There are 7 types of diabetes,  yes 1.5 [LADA] is found in the older people,  1.5 [MODY] is normally found in young people, often found because of sudden weight lose/thirst/peeing

Most people believe that modern lifestyle meat alcohol little exercise etc is OK - I understand that earlier really believes what he wrote and meant well - we will have to agree to disagree until all the MDs and scientists agree...

 

One thing about carbs is taht junk food carbs are bad for you. And too much of anything is bad for you including water...
