replacement of lost driver's license?

Anyone with recent experience of required documents and steps to get replacement for lost driver's license?

Understood can get in Pattaya DLT. Police report of lost license have already

Did this in Bangkok recently.

This is the list DLT said they wanted:
Police report
Residence certificate
Details of lost licence (photocopy, or other info)

What they actually wanted:
To see the digital copy on the phone
A couple of hundred Baht for the replacement (200? 300? I can't remember).

The whole thing took about 5 minutes and most of that was waiting for it to print. 

Sorry I forgot passport & copy was also on the list, but they didn't ask for it on the day.

I don't think Residence Cert was required at Pattaya when i went as it's just a copy of the same licence

did that just today as luckily had photos of my licenses. But still, want to have the hard copy as well, just in case.

 

Seems there is some uncertainty whether residence certificate is needed or not - guess better take the pain and wait a bit in immigration first to get one rather than go DLT without, and wait hours in DLT just to be told that I need that...
