Well, apart from the time when I submitted by mail and never received confirmation.  I had to go to the office and search through a vast manual ledger looking for where they'd received my mail in report.  Total waste of my time.  (Admittedly, this was about 15 years ago, and at a provincial office, so maybe they've improved things since then.)

They put red stars next to some input field, I'm guessing to indicate they are mandatory.  However, forename is not flagged as such, even though it is required.  There are other fields, too, I believe.

I just wonder who actually developed this system.

Did you not use the autofill? (enter passport number select country and press the search button)

You must remember TIT the middle name has no asterisk because the majority of Thais do not have a middle name.

I have Thai friends that entered on foreign passports they must provide TM47's too.

They certainly have. 

You mention CW in OP.

In the return envelop they will also include the photocopies that you posted with the TM47 

 

With online report...assume you used magnifying glass auto fill after entry of name and pp # field

I have rarely encountered an operation which is so utterly unprofessional and generally incompetent

You'll find 99% of issues are due to foreigners Incompetence, not understanding the procedures here in Thailand. 

 

It's quite easy to post negative threads on AN, you'll get lots of likes but it won't make your life easier. 

 

I find dealing with government departments in Thailand quite easy and straight forward once you understand how their systems work. 

 

Maybe Thailand is not for you.

What does this even mean?

 

The system works if you use it correctly. They key points seem to be (as far as I understand it - correct me if wrong anyone):

1/ Have a valid TM30 with the correct address.

2/ Ensure the TM47 is filled in with a correct matching address on the correct time schedule.

3/ Do not have any international travel since the last successful online 90 day report, otherwise it does not work on your next 90 day after return.

It does not. Far from it.

 

There are numerous posts on AseanNow regarding rejection of online report.

Including the list of "conditions" you posted. 

I have 4 rejections from online (CW) despite successful online reporting in the past and as result have done in person.

It's fine till online falls out of love with you.

Yes that'll be it, software and web page issues are due to foreigner incompetence.

Great news it works for you and many others. 

I can assure you it is fractured for many and it's not down to incorrect filing in of the fields.

I have (like many) been rejected even using auto fill after previous successful report.

I was getting my 90-day report rejected without any reason and everything was submitted correctly, first report done in person at immigration, TM30 done.

Then I tried to submit exactly the same data 2nd time and suddenly it was accepted. The system does not work properly or the immigration officers are rejecting it randomly.
