Urologist and ultrasound - Sukhumvit hospital (Ekamai bts)

Backstory, male 40s, testes / balls swollen and dull pains around groin, tried local pharmacy and given amoxicillin / augmentin prescription 1g x twice a day, some relief for first day or two, and third and fourth not as bad, but after stopping seemed to return to full power a few days later, course was only 5 days admittedly.

Week later went to Sukhumvit Hospital at Ekamai BTS and seen a male urologist, urine test clear, examination reveals" mild swelling right testis with tenderness and left variocele gr.2" and got some new medication (more on that below) over 10 days, which is helping or at least same first day relief, and got booked in for a doppler ultrasound on testes - his recommendation for next step, back to waiting room and then presented and signed a piece of paper that I could afford 7800thb price of the ultrasound on my future next visit, I guess that's what they do with non-insurance, but I wondering now with the pharmacy markup below did I have my pants down twice in that visit and only once in the doctor's office? Is the ultrasound price pretty standard?

I got given 10 levofloxacin (cravit brand - thai manufacturer) 750mg tablets, according to moph government website the average price in the market is 14.98baht per tablet - is this up to date?. https://ndi.fda.moph.go.th/drug_detail/index/?rctype=1A&rcno=4500002&register=MUEgMi80NShOKQ==

Sukhumvit Hospital charged 231.24thb per tablet - of course the doctor fee, nurses fee, lab fee, service charge are separate line items. 231.24/14.98  = 15x? 

Is 7800thb pretty normal for ultrasound testes? I guess this won't include doctor fee too?

If sexually active i would have taken antibiotics for Gonorrhoea and chlamydia first, Ceftriaxone and doxycycline, less than 1000 baht. 

 

Sounds very like orchitis, which could be a bacterial or viral infection.......sometimes you just never find out and it sorts itself out.

 

 

Other than the medications, costs seem pretty typical for a private hospital here IF it was a pretty senior specialist. 

 

Medication costs are always greatly inflated at hospitals. Unless a controlled substance is involved, should always buy the meds yourself at an outside pharmacy. 

 

The ultrasound amount seems high to me, in general around 5k is more usual though I have no experience or feedback specific to ultrasound of the testes.

noted about medication. 

 

they had a promotion of ultrasound KUB (kidneys, bladder, ute something) on the TV advertising screen that was much cheaper but the nurse (green uniform i think) pushing the price back in the waiting room after finished with the doc said that didn't cover the testes and this was a different ultrasound hence the price.  seems quite eager to get that signature not sure if any commission at play  🙃

No. You won't get it for this price.

230 B per tablet is several times the price of Sukhumvit pharmacy at BTS Phrakhanong.

It includes the fee for the doctor doing the ultrasound, but not the fee for the urologist.

 I have seen hospital prices,  but cannot remember.  Easily the same you paid.

 

As @Sheryl said, I buy medicine at pharmacies.

Sukhumvit Pharmacy near BTS Phrakhanong is the closest (ie lots of stock, competitive prices) one to Sukhumvit Hospital. Their main branch is at BTS Ploenchit.

I don't think I have ever seen Levofloxacin 750mg in a pharmacy (but I probably never looked). Usually they sell 500mg, 10 tablets about 400 B (generics less than 300 B iirc). Easy to take 1 1/2 tablets.

As ultrsound of testes is not as common as some other ultrasounds,  not generally posted on price lists. But I cannot see why it would cost much more than say carotid ultrasound or  an  echocardiogram (ultrasound of the heart).

As both @Lorry and I have told you, what you are being asked to pay seems high.

 

The doctor fee also a bit high though not enormously so.

 

It is unusual in my experience to be asked to sign for a quote for a simple ultrasound. For surgeries yes, but not just for an ultrasound.

 

But as you have already started the process with this doctor, the larger question is: how comfortable/ confident do you feel with his expertise?

It is not likely to cost you more, and may cost less.

 

Bring all records, test results etc  with you. You do not have to go back to the doctor to get these, just go to medical records dept with your passport and hospital number. Usually free.

Thanks Sheryl, medicine is a lesson learnt, I'll go along as appointment today, the worry tax, but if they stiff me today I'll pop along Bumrungrad, I'm gonna try nail them on the quote too to make sure thats the "maximum" before he starts with the ultrasound. Do they get to just make up prices as they go along? different for every tom, dick and harry?  I'll update for others anyway

Actually postponed it for now, as condition seems to be gradually improving, will see on Monday, they were fine about it, just said to call next week to rebook when ready.

Sorry, no idea.

I quite agree with @Sheryl

They don't make up these prices case by case. You can call a cheap hospital, eg St Louis 02 8385555, and an expensive one like Bumrungrad and ask for the price of an ultrasound of the testis. Either the cashier or the nurse at the urology department should be able to tell you. Make it clear, it's not a KUB ultrasound.

 

I had ultrasound of my testicles recently at St Louis. It was about 4k

He received the quote in person at the hospital and was asked to sign an acknowledgment for it (unusual for just an ultrasound).

Went - worry tax. ~8100baht in end cutting it short the post-consultation out.  From entry from door to cashier in less than 30 minutes. No consultation. Prep work from nurse to angle the bits took maybe 10 minutes etc and ultrasound doctor in and out maybe 4-6minutes.  Very quick and done. 

 

Given the bill the 2x doctor fee of 3040. 1040 for ultrasound fine. and 2000 baht for something else. Obviously the ultrasound (4360) and nurses fee separate. 

 

Said to come back at 4pm to speak to the doctor (another 1600-2000baht), they got told to get fcked, had my pants down enough today. 

 

Asked for clarification on why two doctor fees (so far, a third at 4pm) and havent spoke to a doctor and got given the runaround about thats from the original doctor to order it, wtf paid him on tuesday, then said ah no thats analysis, then said ah no thats IT problem because we are changing systems. Tbf they couldn't explain the charges either.  Ridiculous but done.

 

Its the wild west out there. Going to bumgradrad to figure out the assessment/results.  Will upload receipts laters just incase it helps anyone in future (and to avoid bull<deleted> accusations). 

 

So woul;d of been maybe 10k if spoke to a doctor but after paying for two already wasn't in the mood, ill either bumradgrad or chatgpt it and deal with it. 

 

So would of been nearly 5k in doctor fees alone today if done the course where other hospitals were quoting 2500baht+ (they all said + to be fair, so i guess add 800 doctor fee and nurses fee of a few hundred) 

 

Made my thoughts clear, i might be paying for this myself but with my 10m/100k excess (mtl) insurance you will never get a look in again. 

 

When you're in a rush/fear it is what it is.

Thai medical establishment will always ask you to return for results. If picking up lab tests that's BS but interpreting ultrasound? 

 

Suggest you just go to home country. You're not cut out to navigate Thailand judging by especially the last post .. .which I can barely discern

Whats is your problem? There doesn't seem to be a disagreement here apart from you. It's high, that was said, it was disputed, it's done. Are just awkward with everyone generally? 

Not a Dr or giving medical advice, but it sounds like you're being milked. 

Attend a decent government hospital (I'll suggest Suranaree University of Technology Hospital, Nakhon Ratchasima fits that description). 

An uncomfortable condition. I know someone who had a similar condition diagnosed (including blood tests, ultrasound, medicine) for about 2,000 baht. Surgery was 2 weeks later costing less than 40k baht.

Note it was a similar condition (hydrocele not varococele).

I don't know about test., but I had lower abdomen (bladder and prostate)for ฿2000 and I ask another hospital in Chaeng Wattana that was ฿1900.
