Can anyone advise me on the best options for medical insurance for my thai girlfriend on Samui.

Get her a legal job and she will be insured through her employer.

Other than that, I use the services of a Thai broker in BKK through whom I insured my family, Thai wife, two kids and myself. PM me for details. Very customer service oriented and knowledgeable.

So far my experiences on the island were at an amateuristic and incompetent level with limited options.

Many Insurance companies offer Health/accident insurance in Thailand, there should be some on Island, AIA, AIG, Blue Cross, NZ, Thai Life, etc, if you cannot find one that you like on Island just come inland, Bangkok, pattaya or where ever, they can give her a Health checkup, (Normal stuff/routine check) for STDs, Blood Pressure, Blood sugar, etc, at there cost, most it not all these Companies will insure her world wide. ING Thai is wonderful Health/Accident Insurance company but the insure only in Thailand>POC for ING>66 0 2632 6222
