I printed the affidavit off BKK embassy site, requires legal size paper. <deleted>. I went to Staples (US stationary store), even they do not have the stuff.

Can this go on 8.5x11 or wait till back in Thailand at least a4 - can I get the proper form at the embassy??

Are photos part of the notarization process? Will std passport photo do?

Affidavit calls for references in US, can they be family, relatives? Many last / best good friends travel as much as I do and rarely in the US.
