The new expire date of extended tourist visa is based on last day of old expire date or apply date?

It's been several years, but for me, they added 30 days onto the expiration date of the second 60 day entry stamp, even though I was there a week before it expired.

don't know about that visa specifically, but in five years of going to immigration for different stamps/visa extensions, they have always been based on the expire date of the visa that is being extended... not the application date

Correct, extensions are always added to the expire date. And not based on application date.
