i think i found two forms similar to them in thai; i cut and pasted a translation of the words in a thai search google, then got answers in english/thai...

both forms i got seemed much less 'wordy' then the form we got in hebrew (written of course in high, legalize hebrew and not every day speaking/writing language) so wasnt sure if the form was really what i wanted...

will print out and fax as an attatchment w/o him signing the one in thai, as an 'example' of what form he really did sign.
