Well you're right Kayo, maybe a bit too long, but a story of how many small things, inconsequential on their own, when added together will cause meltdown.

I have read the book 'Lost Moon' or 'Appollo 13', which was a similar story, without the finality of death of course, many small blunders that added up to to a disaster.
