Depending on your age (if 65 or under) I recommend World Nomads. Very broad coverage for a reasonable price. (https://www.worldnomads.com/) I used them for years with no problems. Unfortunately, I am a year past their expiration date. :(

forum sponsor - or perhaps used to be? great company, been doing business with them 10 years. bought a great travel insurance policy from them last year that paid actual cost for everything to the USA. they have several travel policies.

Allianz a big international company with brokers all over. Always used them fortunatly never had to claim.

I just got a policy for a visit to the US...I just got a quote for a 70 yo living in LOS going to Malaysia for 30 days.......50K coverage for 200 usd...and pre-existing conditions are covered for 5K.  Mine was 155 for 27 days with 750K in coverage and a 100 usd deductible....80% of the first 5000...then 100%....outside the US it's 90%......My card says United Health...backed by Llotd's..company is in California.  https://www.visitorscoverage.com/visitor-insurance/liaisoncontinent-a/select-coverage/

If you're covered in the US, and don't plan on going there; you are spending too much.  Perhaps you could give us a ballpark figure for how much you are paying and the deductible.....Blue Cross seldom offers value...it does have name recognition...not necessarily good.  Saw a sign up at AEK Hospital in UTH specifically stating BUPA customers must pay when services are rendered,,,,

Go online and get out the prices based on your own details

 

I am well aware about including North America
