Hi All,

Some help or advice please folks

I recently completed the spouse visa for my wife online, i have paid the NHS levy and visa charge and i have just gone to print the form off to mail to her with supporting docs.

I only now noticed that her maiden name is showing at the end of the form , above the signature box

We successfully applied for 2 visitor visas previously and i used the same visa4uk account for this application, although i did change her name to her married name on the first page (it had defaulted to maiden). So it seems like it has somehow still used the maiden name from previous visas. Her married name appears everywhere else (except the section regarding maiden names of course)

any thoughts on whether this is likely to cause a problem? obviously she will provide marriage cert, name change confirmation all translated etc. I'm just worried as its obviously an expensive mistake if its rejected!

thanks in advance for any thoughts

I dont know if this is any help, because Iis not  the same situation , but I remember when my wife had a green card which had her married name, before she got her citizenship papers  ans US passport, and traveled with her Thai passport which had her maiden name. She will travel with her birth certificate and marriage certificate to explain the discrepancy. She never had a problem and we traveled extensively.

Recently while in Thailand her father passed away,  The airline required a death certificate to make a travel date change with out the extra charge. The death certificate of course showed her fathers last name which was also my wife's maiden name. Again marriage certificate and birth certificate to the res rescue.

I believe, unless you can change the name on the form, the same might be your solution.

I would just amend the incorrect entry and include a note explaining why. Don't think it's something to really worry about as they will be able to see how it was done and just a mistake.

As davidll says, simply amend the hard copy and include a note explaining what you, or rather she, has done and why.

BTW, I assume she has changed her passport. The name on her visa should be the same as that in her passport.
