I have decades long chronic fungal sinusitis ( no cure yet but working on it) and every few years need Sinus surgery, the technical term is FESS ( functional endoscopic sinus surgery). 
Can get it done in my home countries ( I have more than one)  but usually have to go private as wait lists are very long in the public system . 

 

Considering Thailand as an alternative.

 

Has anyone here had this done or knows someone that has and can recommend a surgeon / hospital.

 

Failing that any recommendations of hospitals that have a known good reputation and are not just catering exclusively to rich medical tourists those with unlimited budgets?

I can recommend Prof. Songlot  Aeumjaturapat.

 

He has hours at St Louis Hospitakl, a non-profit on Sathorn road near the Susrasak BTS station. 

Sunday :: 09:00 AM - 04:00 PM
Thursday :: 03:30 PM - 07:30 PM

But call to confirm in case it has changed.  02-838-5555 (website is only in Thai)

Appointment not needed but get there early as it is first come, first serve. Register for a hospital number on the first floor first.

 

He can also probably be seen at the Chulalongkhorn after hours clinic (13 floor OPD building, from 4 PM) which will be even cheaper, but less convenient (have to go in person to make an appointment, there may be a wait list etc).

 

(He's also at Bangkok Hospital, but that's exactly the type of hospital you describe wanting to avoid).
