Has anyone used a fit to fly certificate received from QuickMD or any other similar online service where you can get one from speaking to a doctor online over videochat? Will that work or do I need to actually go in and see a doctor?

Yes, video or even telephone conversations are fine to get a fit to fly certificate.

I got my fit to fly certificate from medconsult! Tried and tested, and soo simple - You pay 400 baht, you receive a link to fill in an online form (which took me 30 seconds) then receive the fit to fly by email! There's a 2 min video consult after to approve it.
