Where has someone gotten a vaccine passport? Was it difficult?

I am scheduled to get my first shot of the AstraZeneca vaccine tomorrow. After I am fully vaccinated, I would like to apply for the vaccine passport as rolled out in April. This seems to be available from a few different places, such as the Bureau of Communicable Diseases at the airport, the "Travel Medicine Clinic" and the Institute for Urban Disease and Control.

 

I'd like to know if anyone else has gotten this document yet, where you got it and if it was a hassle or not. I'd like the passport as to have the most official possible document to show that I am vaccinated.

You get a small sticker that can be put in your passport. That is in Phuket. Dunno about other places.

I would never myself affix a sticker in my passport.
