Hello, Wed I will extend my Non-O Retirement visa. The forms they gave me at CW included the attached form which I've never seen before. Who fills out this statement? My landlord lives out of town. Can anyone attest that I DO LIVE in my own condo?  Someone suggested you can put Self and fill it in yourself. Is this correct?

 

Also, to be clear, now they only need: TMP7, photo, hand drawn map, bank statement and attached form. (At BKK CW)  Correct?

you fill it out yourself
It is a self guarantee form
They have been handing it out for 4 or 5 years (really they were just handing it to you when you applied and had you write your name on it and sign it, along with the overstay penalty form, the terms of your extension form and the proof of funds form

Ah ok, thanks!  I never really noticed it last year or the year before. I sign whatever they put in front of me. HA

These forms (think 3) are just "acknowledgment" forms for overstay and such.

Here is paste I made from a previous thread re same forms.....

I just signed them.

io seemed to have no interest in them.

Read similar reports to my experience.
