Lol, Tramadol does nothing for me either, whatever the kind of pain (muscular, headache etc.).

 

Kratom can make for a good night's sleep without pain. It is not harmless though.

Tizanidine (Zanaflex)?

 

I have used it as a pain killer and sleeping pill at the same time. It is classified as a muscle relaxing drug. No idea whether it's available in TH. If it is, then surely not OTC.

I have bought Tramadol over the counter in Chiang Rai, after explaining to the pharmacist I needed it for back pain.

 

The OP needs to experiment with the various pharmaceuticals to determine which  are most effective. We all are different, morphine and marijuana do absolutely nothing for me.

Arcoxia works great for certain types of pain.  But I noticed my pulse goes way up when I take it.  So I stopped using it.  It was a reformulation of Vioxx which worked great too but was taken off the market because of increased risk of heart attacks.

for pharmacy that sells tramadol with no questions asked, looks for the ever revolving clientele of boys on  motorcycle out the front, 

some even sell soft drinks for the convenience too

Quite the opposite. It is a well respected large private pharmacy on a small street near the market. No mobo yobbos.

Actually tramadol doesn't do anything like that for me. Stops my shoulders from aching at night, that's it.

Actually 99% of the time it's not as bad as I fear and I struggle thru. I really don't like taking medicines.

It's more the peace of mind knowing it's there if I need it. 

I remember in the past.. Some doctors private offices are able to prescribe xanax. Over the counter medicine is crap here. Some pharmacies will have lorazepam which is an offshoot of valiums. They can relax your muscles and help some for this. Just a matter of checking around. Tramadol and Ultracet work somewhat. but depends on how bad the back pain is and how many milligrams you take. But those are not ever-the-counter meds.

I have no  cartilage in my left ankle, big pain sometimes. I was prescribed tramadol, but using this I cannot eat for two or three days after. I just use paracetamol and get by, medicines scare me these days.

Buy generic etoricoxib.  Brand name Arcoxia (same ingredient) is overpriced.  90 mg once a day to start.  But, don't use it for more than 4 or 5 days in a row.

Several posters advocated benzodiazepines like lorazepam or valium.

Whether these are useful for back pain is very much in doubt,  most doctors don't think so (they will make you tired,  though,  after 10 years of back pain 2mg valium may give you the first night you sleep well).

They are difficult to get in Thailand, hardly ever OTC (illegal), and I would not want to be caught with illegally acquired benzodiazepines.

Easy to get on hospitals if you have a reason for it. I got 10 5mg for sleep when flying, also for my back pain when I told her I needed to just take a time out once and awhile. 

 

It might help if you use one hospital for regular check ups and have an dr

Ultracet is Tramadol plus codeine.

 

Both are sold OTC but only in pharmacies with "real" pharmacist always on duty and willing to keep the required records.

 

Some people (me included) experience a severe "dysphoria" (generalized icky/weird feeling ) from it as well as nausea and constipation. I get all 3 so have to be in serious pain without other recourse to take the stuff. But others do fine with it, some people even like it and get somewhat addicted to it.

It is in fact OTC in Thailand.

 

Brand names

 

Tonolyte (locally made)

Tidar (locally made)

Tizan (Indian generic, Sun Pharma)

Sirdalud (import)

 

It is a serious drug, mainly used in conditions like multiple sclerosis and spinal cord injuries where there is severe  muscle  spasm,not prescribed for your average stiff back.  It has some very serious potential side effects, contraindications and  drug interactions so research carefully before using.

I take co-codamol occasionally in the UK but for some reason always thought it was banned/not available here in Thailand. Can you remember the strength? I find it far more effective than the Ultracet (which does not contain codeine, but paracetamol/acetaminophen and tramadol).

I had an awfully bad back 2 months ago. ( L5S1). While waiting for the cortisone injections rdv at the hospital , I asked my specialist for Tramadol  to get me over the bad days. He gave me Tramadol 100. Wow, I was really sick after and felt terrible, but they certainly stopped the back pain !!  I couldn’t use them . Were they too strong, why was I so ill after them ? 
i suffered with heating pads and paracetamol.

That particular brand is not here but there are brands of paracetmol + codeine that are. Most have just 15 mg of codeine, plus either 300 or 500mg of para,

 

Strictly controlled, have to get from a hospital or clinic.

How long did you take them? 

 

Some is allergic and sensitive to opioids and cant take them at all.

100 mg is a very large dose.  50-75 mg would likelyy have also helped the pain with fewer adverse effects.

 

That said, tramadol does have some nasty side effects, as I mentioned in my prior post. Affects some people more than others.

Sheryl please, I had a really bad reaction to Tramadol 100. I really can’t take them. What can I ask for to really help without the side effects please ?   I think I had  paracetamol had something with paracetamol and codéine that worked . Can’t femme tho.  I have L 5 S1 and sometimes feel as tho my right hip blocks which scanners say it doesn’t.

Hi, took about 5 in all over 8 days. Just wasn’t worth the stomach upset.

First of all - Tramadol 100mg is a huge dose. 

 

A single tab of Ultracet , by comparison, has only 37.5mg of tramadol in it.  Might be worth a try.

 

Para plus codeine is certainly better but you can only get it from a hospital or clinic and Thai doctors are hesitant to give  for chronic pain.

 

Have you tried an NSAID? (Ibuprofen/diclofenac/celebrex/arcoxia etc)? Do you have any contraindications to them?

 

Have you tried drugs in the gabapentin/pregbalin family? (Anti-seizure med often used for nerve pain). There is actually a new generation drug in that class which is much more specific to, and efficient for,  nerve pain and has fewer side effects. Brand name Tarlige, but only available from hospitals and only really up to date specialists have it (see below).

 

Most important:

 

1. Have you seen a really good spine specialist? (Only a handful such in Thailand, all in Bangkok). I have/had L5-S1 issues too, but opted for a permanent solution and am now pain free. Not all people's problems warrant surgical intervention, and not all patients are good surgical candidates, of course. But I find there are a lot of people in Thailand with conditions that can only be addressed surgically and who have never been properly evaluated/diagnosed.

 

2. Have you ever had an epidural steroid injection? Related to #1 above, of course, as needs both a spine specialist and good pain specialist to know (a) if it will help and (b) if so, where exactly to inject.

I would say that ibuprofen is just as good as tramadol (for pain).

I would never take Tramadol for more than....maybe....

One day.

Tramadol is a strange drug, in that there are 2 components to it....

a. one for pain

b. but, also it affects mood

Be darned careful with it.

 

I would not recommend it to anyone other than my worst enemy...to be used more than a week, in fact.

This is why I say, just as you also said:  Beware of this dual action with Tramadol.  In my view, this can have some of the same effects as benzodiazapenes or maybe antidepressants.

You can research this for yourself.

This is my theory, anyway.

And so...

We all recall Jordan Peterson's experience with Xanax, etc...haha....

Xanax had him crying on the internet, while spaced out on benzos...

 

Therefore....Only give this stuff to your worst enemy....in my view....

 

 

Again: Ibuprofen is quite adequate for most pain.  Also, learn to Take the Pain!  Because, it is the people who opt for opioids who are the most likely to end up dying from Fentanyl.

 

If you go onto sites at places like the Taiwan University Hospital website, you might be able to get info on pain meds advice.

Unlike the USA, Taiwan does NOT give out dangerous pain meds like they do in America.

It's not wise to take opioids, including Tramadol.

(For cancer...then fine, as well as other similar circumstances.)
