Here is link to form some immigration offices use. CW being one example . Here is link.

At bottom of form you will find list of docs to provide.

There is no charge for transfer of visa info and any Stamps.

Remember to update new pp # with your bank

Why

Well the last time I switched US passports I was asked for the old one at Thai immigration. I believe it was because my new passport was clean except for a Extension of stay in Cambodia, and showed no ins and outs.

Whatever, they took both passports, typed for a while, stamped my new one in and gave them back.

It costs nothing to carry the old one one more time.

The OP is living in Thailand on annual extensions from Non O.

After he attends immigration for transfer of visa information and any relevant stamps ...eg a reentry permit then for future travel he does not need to travel with both passports.

Thats fine, I take the position in life that preparation is the key.

It cost me nothing and took up no space to take my old passport along to avoid any potential hassles or questions from a stressed or ignorant or confused or racist IOs at the border.

I offered my suggestion as a help to the OP or anyone else in the same situation. People can do as they chose. My experience has been over the years that it avoids hassle, as about 17 years ago, I was also ASKED if I had my old passport with me (and I did) at Swampy. They didnt look at it though.

Of course they questioned it, you are supposed to update the new passport - it's not rocket science to think that one thru.

If you are close to your IO I would suggest giving them a visit to find out exactly what they require, we all know requirements differ greatly between IO’s.

I renewed my passport recently and visited my IO to enquire as had heard about bank book and 12 months statement ( i think was mentioned somewhere ).

To my surprise they wanted photocopies of all stamped pages of old passport and photo page of new, together with application form only so did it there and then, took about 15 minutes.

This was Ayutthaya but just an example of how the requirements may differ.
