Question on Thailand Digital Arrival Card

Hello

 

I am a British expat living in Thailand on Retirement Visa. I have a multiple entry.

 

I have to visit Singapore for a week, to come back I believe I need a Digital Arrival Card.

 

I have had a look at the form on line.

 

It says " Applying for " and only allows a choice of 30 Days Single Entry. 

 

As there is no other choice should i use this??

 

Ideal if anyone has experience of this advises.

I have done several of these and don't recall seeing that question.  Where are you looking at the form.  The only place you should go to complete it is here;
