My friend has an infection down there.

        The only advice that should be given on this thread is to seek professional medical advice, ie take her to a doctor kicking and screaming if necessary End of story even the OPs request / idea re self medicating with antibiotics is extremely unwise if not bordering on stupidity and should be called out however well intentioned it might be. Misuse of antibiotics is the main reason we are becoming plagued with resistant infections. 

        Furthermore any sexually active woman should be having a regular smear test it will take a lot more than a pessary to treat cervical cancer. 

   No I am not a doctor just an ex construction engineer with a bit of common sense   tell me i'm wrong

You can go to a reputable Pharmacy ( one of the bigger ones ) and speak to the pharmacist in confidence.  Many can speak and understand English well enough to hear the symptoms and offer a treatment. They are just or better qualified than Doctors in Pharmacology.

In fact people in the UK were advised on this approach instead of flooding the Doctor's surgeries .

Another thing you can do is buy a swab ( a long think stick with a cotton wool bud at the end), if there is a discharge, get her to take a specimen and take it to a private laboratory, ask for “ culture and sensitivity “, you should get the what type of organism there is, then to the pharmacy and get the correct antibiotics.
Or try what Sheryl has suggested, it might work.

If it recurs then you will need to establish whether it is an infection that needs antibiotics.

Doesn’t sound serious though.

Also bathing the area with good antiseptic could help tolerate the odor.

Of course, it might not be an STD but could be a UTI (bladder infection).  I have E.Coli bacteria UTI on regular occasions which makes my pee smell like bad fish (and the smell lingers even when I'm not peeing, so don't stand downwind...).  But of course best to speak with a doctor.

Agreed.

Usual dosage is 400mg 3 times a day, with a bodyweight of 40kg you can use 250mg 3 times a day or 400mg 2 times a day.

No alcohol when taking metronidazole!

5 days is a very short treatment,  7 days better. 

 

Clindamycin is an alternative. 

Canesten vaginal tablets (3 nights if it's the first occurrence,  later 6 nights) are not so much an alternative but an additional treatment

Very true.

A vagina should never cleaned inside with soap (all Thai girls do this), or even water.

And all this scented chemistry is even worse. 

 

Right now,  those big billboards on the BTS advertise vaginal soaps...omg

Think they call it tok khao - extremely common and treated with suppositories. Could easily be misunderstood to be an STD.

As sheryl has said, this is usually not the recommended treatment (no effect against anaerobe bacteria) and it can make things worse (cause fungus) 

 

I must admit,  I have seen guidelines where this was included,  and I once have seen a girl who was successfully treated this way.

Since Sheryl is not a qualified doctor advices are questionable.

However, nobody shall give long distance diagnosis unless you got evidence. 

So, not having more than your statement my idea of treatment is based on my experience only.

If Sheryl is not a qualified doctor, she is extremely knowledgeable.

No, I'll always remember your rudeness.

Not only that she needs to consult and get the blood/urine testing,  but OP also if  having any intimate unprotected interaction, even if only Oral. 

 

And re do the Hiv Tests again in 3 months just to be on the safe side. 

 

However what is described could be a simple issue of (BV) bacterial vaginosis caused by an excess of the Gardnella vaginalis (GV) that is a sort of auto defense bacteria in that private part that is necessary to keep it infection free. But when the GV bacteria is in excess, it can lead to a BV. But this is plainly theory and absolutely not any kind of medical advice to take or consider. Only the medically performed tests described above should be followed. 

Just go to an international hospital. She can find out within an hour what her problem is & receive the correct drug to cure it.
