On the VAF1 application form question 4.9 to 4.12, we are unsure of what to put. The reason being my G/f is unsure of the exact date the previous application was made and has threw away the papers!

Is it best to include a cover note explaining the circumstances with the rest of the application?

In the back of her passport there will be a stamp giving the date of any previous applications to the British embassy, with a line drawn through if the application was unsuccessful.

If she has had to renew her passport since then and does not have the old one; give approximate dates. It will all be on file at the embassy anyway. A covering letter explaining that she cannot remember the details and no longer has the records or old passport is a good idea, but not essential. Or she could put the explanation in section 10.
