Yes... there is an area which asks for the visa number, you will need to fill it in.

I have the same visa, you will have to fill in form, as it is then stapled into your passport alongside the entry stamp. Make sure that you receive the correct stamp, the first time I used my one year multi visa I entered at Chiang Mai and the official stamped USED over my nice new visa and put me down for a Non Imm B, which I didn't realise until later ... at least he got the length of stay right at 90 days!. I had to see immigration at Don Muang when leaving to get them to amend his error, which they did within minutes, otherwise I'd guess there would have been problems coming back. By the way you don't need to get a re-entry stamp/visa with a multi O - which you do require if on a one year work permit visa ... just that I had been told by some people that this was required.

You need a re entry permit to keep a current stay or extension of stay alive. You do not need it for a multi entry visa type 90 day stay if your visa (the stamp from a Consulate) is still valid as you can use that for your next entry.

You hope to receive a stamp for 30 days after your arrive and present that landing card so the answer is no you do not fill in information you do not have. Your entry will be without visa.
